From what I understood,

1st time: he kills the dude and the handle breaks so he replaces the handle (same axe head though).
2nd time: he kills the thing and the head breaks so he replaces the head.

That means it is a brand new axe that had none of it's parts in killing the man. If, for argument's sake, you say that the head that was stitched on is the monster's head (the one he just killed), then technically the handle of the axe would still be the same as the one that it was killed with and he mistook the new head of the axe as being the same as the old one due to it being of the same cheap kind so they'd probably look identical. You could also argue that since its the same generic, cheap axe that it ends up looking the same as the original and then the guy wouldve mistaken the axe as the one that he was killed with.

This applies same principle as the story with
U have a boat made out of 100wood pieces
Each year one wood piece looks older and u replace it puting the replaced piece aside
After 100 years u replaced each piece of the boat

With the pieces u put aside u build back the boat
